Updated:

To help us process your first payment, you should upload all requested documents to the Initial Grant Requirements form, which we’ve published to your account in the online grants portal. Log in and go to the ‘My Tasks’ section to find the form. Once you have completed all the information you’ll be able to submit the form to Youth Music.

1. Signed funding agreement with Youth Music

Please upload a signed copy of pages 1-5 of your funding agreement, which has been sent to you by email.

Two people from the organisation must sign your funding agreement. These must be authorised by your organisation to enter into agreements on behalf of the organisation.

You may find it useful to download a free version of Adobe Acrobat Reader: Adobe Acrobat Reader DC, free PDF viewer download. This will enable you to view, sign and save PDFs.

Watch this short video about how to sign and complete your funding agreement.

2. Completed authorised signatories mandate

You need to complete the authorised signatories form and upload a scan or photo.

This form tells us which members of your organisation are authorised signatories. These are the people we expect to sign the funding agreement and the declaration statement on grant reports and expenditure.

The declaration at the bottom of the mandate must be completed by the Chairperson, or head of your organisation or a Director. It is important that a senior member of your organisation is included because the ultimate responsibility for the programme lies with them.

3. Completion of bank details

The account details must be for the lead organisation’s bank account. We cannot make payments into an individual’s or any other organisation’s bank account.

You’ll need to enter all account details into the online form, which includes:

  • Account name
  • Bank name
  • Bank account number
  • Bank sort code
  • Payment reference

4. Evidence of your bank details

You’ll need to upload evidence of a bank account that matches the information given above. This could be a scan of a paying-in slip, blank cheque or bank statement showing your organisation name, sort code and account number.

If obtaining one of these documents is not possible, you can request a bank details form from us which you can complete and take to your local branch to be authorised with the bank’s stamp – showing that the bank confirms the account is registered to your organisation.

5. Advice and guidance notes (if applicable)

Sometimes we provide advice and guidance notes that we think will strengthen your programme or organisation. These are informed by our assessment and decision-making process.

We will ask you about them in your grant reporting, so It’s worth keeping a record of them. However these notes are advisory not mandatory, so it’s your choice whether to act upon them.

6. Evidence of how you have addressed any special conditions (if applicable)

The online form will list any special conditions specific to your grant. These are requirements that must be addressed. You can respond by sending us copies of the required information or writing your response in a Word document and uploading it to the online form. If you have already responded to a condition, please indicate so in your response.
